2013 ISSA Moutaineer Classic - Entry Form

Please include our team in the 2013 Mountaineer Classic Tournament to be held on July 19-21 in Morgantown, WV. Deadline for entries is July 6, 2013. Rosters should be submitted with your entry form and fee (signatures can be added at the event). Enclosed is our check for the non-refundable entry fee of $400.00. All checks should be made payable to I.S.S.A. in US dollars.
